If you're a qualified vendor and have completed the PPI process, you may obtain your code & information for your PPI Scores. You may only make a request for your company (not any other). PBSRG will verify all information submitted.

If your company would also like to obtain codes for their key individuals, please list the names below. Request up to 5 at one time.
